Who are we?
At Mercy Hospital Grayling, our services consist of management with Coumadin (blood-thinning medications prescribed by your physician), drug interactions that may interfere with your drug therapy, dietary education and surgery planning. Your pharmacist managed clinic is operated and managed by Michelle Sacks, PharmD. We are conveniently located across the hall from the pharmacy department. We are more than excited to welcome you to our clinic!
Why use our services?
Studies show that pharmacist involvement in patient care and medication therapy, especially with blood-thinning medications that you may start or are on currently, can greatly reduce the number of yearly emergency room visits, risks of bleeding as well as increase patient education. At our pharmacist-managed clinic, you will be able to understand your medications more clearly and learn the importance of taking them daily. Michelle is here to serve the community with any healthcare related questions that you may have regarding your medication.
Who is my pharmacist?
Michelle Sacks, PharmD is registered with the state of Michigan as a pharmacist and completed a post-graduate residency program specializing in ambulatory care services in Florida and is in the process of becoming certified through the University of Southern Indiana. Michelle was born and raised in Florida and has recently moved to Michigan to establish the clinic in Grayling. Michelle is dedicated to her patients and upholds the Christ-like model of the hospital.
What is expected of me, the patient?
As a patient of the pharmacist managed ambulatory services (PMAS), I, the patient am expected to keep my appointments, take my medications as prescribed daily by my pharmacist and physician, notify the pharmacist if new or old medications have been started or stopped and inform the pharmacist of any other medical events planned or scheduled in the near future. I will make a point to understand the importance of taking this medication and the consequences if I do not take it.
What do I need to do in order to be seen in the clinic?
You will need to have your physician enroll you into the clinic. Simply call our clinic at (989) 348-0809, or call your physician’s office requesting to be enrolled into our clinic. Once all of your paperwork is complete and turned into the clinic, we will contact and provide you with information that you will need to bring before your first visit.
