Benefits Plus Package
Benefits Plus is a “flexible benefits” plan that allows you to customize your benefits with choices in medical, dental, vision, life, accidental death and dismemberment, reimbursement accounts, eye care and dependent life insurance coverage. Costs are determined by the options you select.
Medical Care Options
All Munson Medical Center Employees, including residents, contribute to the cost of medical care benefits through payroll deduction. Currently this amount is set at 18% f the premium cost.
Option A Priority Health:
$100 individual / $200 family deductible; limited co-payments, includes well-care and preventative coverage.
Option B Point of Service:
Out of Network services with $200 individual / $400 family deductible and 20% co-pay. All approved in Network services - same as HMO.
Option W (Waiver) - No Medical Coverage:
May select if covered by another source (such as spouse); will receive bonus-waiving coverage.
Spousal eligibility form must be completed in order to qualify for Munson coverage.
Coverage effective beginning day one of employment.
Dental Options
Option A:
$1,000.00 annual coverage per plan member. Pays 100% for preventative care; 50% for x-rays, restorative and prosthodontics; and 50% for orthodontia for children under 19, with lifetime maximum of $1,000.00 per dependent child.
Option B:
Covers same services as above, but with $1,500.00 annual coverage per plan member and coverage at 75% for applicable services other than preventative; same coverage as above for orthodontia.

Optical Discount Plan
Plan available to employee through Eye Med Vision Care. There is a one-time payroll deduction that covers employee and family.
Life and Accidental Death & Dismemberment (AD&D) Options:
Coverage at one times your annual wage is provided. Four additional options for combined term life / AD&D coverage may be available. Additional AD&D options are also available.

Reimbursement Accounts
Health and dependent care reimbursement accounts allow the use of pre-tax dollars to pay for qualified expenses, thus providing tax savings.

Dependent Life Insurance
Employee purchased coverage of $15,000.00 for each child and spouse is available, and may be purchased through the convenience of payroll deductions.

Long Term Disability (LTD):
Residents are provided with a benefit of 60% of their monthly wage. This benefit has a six-month waiting period per disability. Long-Term Sick time will be paid out during this time.

Munson’s Retirement Savings Plan (RSP):
Under Munson’s Retirement Savings Plan you will receive a match. You may also receive a 3% annual contribution, whether you contribute or not, provided you were employed the full calendar year and you were paid for at least 800 hours during the calendar year. You may refer to your Summary Plan Description for full details.

Holidays
Residents are scheduled on a rotating basis for duties during regular holidays. Please refer to policy #34. 008
Short/Long-term Time Off:
Provides for paid time off for absence due to illness.
Short-term time off allows for up to three days per year for personal or family illness. These days may be requested by a resident with approval of the Director of Medical Education or the Residency Program Director.
For long-term time off, residents are credited with 1040 hours at the beginning of each year, to be utilized for extended periods of personal illness.
Accreditation and board certification requirements necessitate that each resident complete all elements of the training program. Therefore, time away from the program due to illness, in excess of three days per year, will need to be made up in order to complete program requirements. This may require extension of the training program.
Courtesy Allowances:
Are provided to Munson employees and legal dependents for pharmaceuticals.
Other Benefits:
Are also provided, such as Liability and Travel Insurances, Employee Assistance Program, Direct Deposit, Leaves of Absence, Employee Health Program and Child Care Center.
Moving Expenses
Up to $500 of moving expenses will be reimbursed to new residents entering the program.
Meals
Residents are issued a separate "swipe" card, which can be used for meals WHILE ON DUTY only. This privilege is extended only to the resident, not family members. Graduate Medical Education is billed for these meals each month. This same swipe card would also be accepted at the MMC Gift Shop and Ambulatory Pharmacy, but it is not to be used for this purpose.
License Fee/Membership Dues
First year residents are provided with an educational license at the hospital’s expense. A full license is provided at the hospital’s expense once second and third year residents have met all appropriate State requirements.
All DO residents are to maintain AOA membership at all times and will be reimbursed by the program for the cost of this membership. All DO residents are to maintain membership in the ACOFP (there is no charge for this membership). All DO residents are to maintain membership in the AAFP and this cost will be taken from your CME bank.
All MD residents have the option to join the AMA. All MD residents are to maintain membership in the AAFP. The Department will reimburse the cost of one of these.
Liability Insurance
Medical liability (malpractice) coverage is provided by MMC for each resident while performing residency duties. This liability coverage does not extend to moonlighting, consulting, or other activities outside of the scope of the residency. It is the resident’s responsibility to arrange liability coverage for all outside employment activities.
Miscellaneous
ACLS and Neonatal Resuscitation Course
Fees:
All residents are required to maintain current certification in BLS, ACLS and Neonatal Resuscitation. Residents should acquire initial certification and be current in BLS and ACLS prior to starting program. Neo-natal resuscitation will be completed during orientation week. Re-certification courses are provided at MMC at no cost to the resident.
Lab Coat Allowance:
A $70.00 lab coat fee is provided for each year of the residency program. The Program Coordinator will assist in the order of the lab coats for the residents.
Pagers:
All residents are supplied with pagers and are required to carry them while on call and on duty. Residents are responsible for the pagers with the understanding that if the pager is lost or damaged, replacement will be at their own expense. If a pager is malfunctioning or not working accurately, a loaner pager can be given to the resident by the MMC Switchboard (but you must have the GME Coordinator submit a request via GroupWise) until their original pager is repaired.
B. Electives and Out Rotations
One month of the PGY1 year, two months of the PGY2 year, and two months of the PGY3 year are allocated for elective rotations. This time is to be arranged by the resident with the approval of their Faculty Advisor and the Director of Medical Education. All elective rotations must have clear learning objectives and an evaluation procedure in place before they will be approved. Residents will be expected to continue their normal lecture, call, and continuity of care responsibilities during their electives unless they are on “out rotations”. Requests for elective rotations must be submitted at least 60 days in advance. If an elective has not been arranged 45 days in advance, the resident may be assigned to full time duty in the FPC.
Up to one month of out rotation time per year during the PGY2 and PGY3 years may be arranged to meet individual training needs of the resident. During out rotation the resident will be excused from their normal FPC and call duties. The Director of Medical Education must approve all requests for out rotations in advance.
C. Expense Reimbursement Protocol
(CME / Travel for each residency year is as listed under Section A: Benefits Summary)
- Residents will be reimbursed by the program for approved expenses relating to travel, medical education, and other reasonable expenses. This allowance, with yearly limits as noted above, cannot be carried forward from one academic year to the next.
- All requests for reimbursement must be accompanied by proper documentation including:
- Receipt
- Statement of the purpose of the expense including the relationship to the residency program.
- As a general rule, only those items, which would be considered normal business expenses, will be considered for reimbursement.
- Allowable expenses include the following:
- Travel to and from:
- Required SCS educational meetings
- Distant rotation training sites
- Medical Education Courses
- Board Examinations
- Registration, food and lodging expenses for medical education conferences and board examinations. (These items will be reimbursed with limitations as required on Munson Travel Reimbursement Policy #001.023).
- Medical texts, journals or software to be utilized for residency educational activities or patient care activities.
- Professional association dues.
- Other activities that relate to the educational or patient care duties of the resident within the program.
- Capital equipment, which will be retained by the resident at the completion of training such as computer hardware, is not eligible for reimbursement.
